Two Kodiak candidates met their main goal against aflibercept in the DAYBREAK Phase 3 trial, and the company plans a multi-indication filing with the FDA in the fourth quarter.
Kodiak Sciences said its DAYBREAK Phase 3 trial in wet age-related macular degeneration met its primary endpoint for both drugs it tested. Zenkuda and a second candidate, KSI-501, were each non-inferior to aflibercept, the drug Regeneron sells as Eylea, on vision gains at one year. The p-values were 0.0007 for Zenkuda and 0.0036 for KSI-501. KSI-501 also met a secondary endpoint on retinal anatomy, with a p-value below 0.0001.
Kodiak shares rose about 160% in afternoon trading, on pace for their biggest one-day gain on record.
The dosing interval
Matching Eylea on vision is the requirement. The dosing schedule is the selling point. In the trial, 54% of patients on Zenkuda reached a 24-week interval between injections, or about six months. Eylea patients typically receive a shot about every two months.
For a disease treated with injections directly into the eye, the difference is large. A patient who needs six injections a year on a two-month schedule could need two on a six-month schedule, with fewer clinic visits for patients who are often elderly. That is the ground on which newer drugs compete in a market Eylea has long dominated.
The limits of the result
Non-inferiority means the drugs were shown to be no worse than aflibercept on vision, not better. The 54% figure also means that just under half of Zenkuda patients did not reach the six-month interval. The full data, including safety and the share of patients at other intervals, will matter for how doctors and insurers weigh the drug against existing options.
Kodiak said it plans to submit a biologics license application covering multiple indications to the Food and Drug Administration in the fourth quarter of 2026.
Two readings
One view is that the stock is pricing a drug that can take share from an entrenched franchise on convenience alone, now that efficacy is shown to be comparable. The other is that a move of this size prices in approval and commercial success before the FDA has even received the filing, let alone reviewed it.
The fourth-quarter filing and the full data presentation are the next milestones. The FDA's acceptance of the application would start the formal review clock.
